A custom database solution will be designed to exactly mimic your processes. Your data will be stored in your corporate database (usually Oracle or SQLServer) so that it is well protected and backed up. The front end will usually be web based so that it can be rolled out on your intranet with little overhead. If large amounts of data or complex calculations are required then a client / server approach may be preferred.
Examples of such databases would be Project Scoping and Initiation, Health & Safety related databases and Data Storage and Analysis.
